Request Forms

You may also request scans and items by mail, using the forms on our website. You will receive a copy of your request(s) by email. We will email scans and send mail requests by Library Mail.

Curbside Pickup Request Form
If you are uncomfortable or unable to enter the library, you can submit curbside pickup requests using this form.
Mail Request Form
Use this form to request library items if you live more than 50 miles from the seminary and within the United States.
Scan Request Form
If you are an online or distance student, use this form to request scans from items that the library owns. Please note, we cannot scan entire items in most circumstances.
